Complex care provides a citywide nursing service to adults and children in their own home. The service provided may range from a one-hour visit to 24-hour care.
The health element of care is free for all patients and all care is free for terminally ill patients who meet specific criteria, as determined at assessment.
All care packages are reviewed at least every six months and modified according to the patient’s needs.
Who is it for?
The service is for people registered with a Birmingham GP who have long-term illnesses, severe disability or who are terminally ill. The service operates a fast-track discharge system for terminally ill patients. It should be noted that the service does not take out-of-hours referrals or run an emergency service.
Adult patients must be assessed as meeting the relevant continuing health care criteria or require a joint social/health package of care.
Care provided at home must be deemed appropriate from a medical and nursing viewpoint and must be agreed by the patient’s family. Appropriate facilities for both the patient and staff must be available in the home prior to discharge.
Who delivers it?
The service is delivered by a team of nurses working with the integrated multidisciplinary teams as appropriate.
Referrals by GPs and Healthcare professionals
Referrals are accepted from discharge liaison nurses, community health care co-ordinators, community children's nurses on 0121 255 0744.
