The service provides management of chronic kidney disease (CKD) where kidney function is moderately or severely reduced. Services offered include patient and professional education.
Who is it for?
The service is currently offered exclusively in the Central and West locality and therefore only patients registered with a Heart of Birmingham GP practice may be referred.
Normally, patients will have moderate or reduced kidney function (KDOQI stages 3-4). There is also end of life care and support available for stage 5 patients who have opted for conservative treatment (rather then undergo translplant or dialysis).
Who delivers it?
The service is delivered by consultant-led, multidisciplinary clinics, in some instances combined with other vascular services.
