Diabetes Service

The diabetes service offers a comprehensive service for adults with diabetes.

The range of care provided includes group education sessions delivered in a number of commonly-spoken South-Asian languages. Domiciliary visits may also be undertaken in specific cases.

The service provides a multidisciplinary team approach, with clinics staffed by consultant diabetologists, specialist nurses and specialist diabetes dieticians. There are also virtual clinics – where a consultant and diabetes specialist nurse attend the GP practice and offer advice, education and management plans for identified patients by the practice.

Key services

The team provides highly specialist treatment advice and training in:

  • insulin initiation

  • assessment and titration of oral hypoglycaemic agents

  • blood glucose monitoring

  • carbohydrate counting

  • assessment and transfer of insulin regimes

  • initiation of GLP1 analogues
  • patient self-management of hypoglycaemia

  • weight-management

  • one-to-one telephone follow-up support.

  • DAFNE for type 1 diabetes

 

Who is it for?

The team's aim is to offer support to all adults registered with a Birmingham GP who have Type 1 and Type 2 diabetes and who are at risk of complications, whether that is due to a raised Hba1c (click here for further explanation of Hba1c), weight management issues or problems with injection sites.

Pre-diabetes patients can also be referred for the DESMOND education sessions.
