The PLN Service offers
Effective communication between primary, secondary and tertiary care, which facilitates continuity of care for the clients/children and their families.
Facilitates follow-up of children attending Accident and Emergency Department, in order that ‘best’ care is given.
Aims to reduce the accident and emergency attendance of children by education and liaison so that care is given in an appropriate setting (i.e. GP, Community).
The promotion and advance continuity in Child Protection cases by liaising with hospital and community staff to facilitate decision making and formulate care packages.

Specialist advice regarding care/services/resources available for children in the community.

A resource available for parents, nursing and medical staff.

Health visiting advisory service available within the hospital.

Close liaison with Community Health Visitors and School Nurses across the city and nation-wide.

Health Promotion activities within the hospital and community setting e.g. Child Safety Week.

Teaching across a multidisciplinary field.
When is this service provided?

Attendance at the Accident and Emergency Department.

Admission to a ward.

Repeated accidental injuries/unexplained injuries.

Inappropriate attendance at A&E for minor ailments and/or recurrent hospital attendance.

Long term chronic illness requiring input.
