Health Facilitation Service

The team consists of registered learning disability nurses with a wide and varied background in meeting the health needs of this client group, with many years of experience.

We operate an open referral system and aim to assist health professionals in the acute and primary care sector across the whole of Birmingham in the following ways:-

  • Signposting to other services, in particular the learning disability community health team, a comprehensive multidisciplinary service serving all of Birmingham. Services offered include Community Nursing, Speech & Language Therapy, Dietetics, Clinical Psychology, Occupational Therapy and Physiotherapy and Psychiatry.

  • Providing expert advice to clinical staff including information on best practice, specific health problems more commonly found in this client group,and help with ethical dilemmas. We can also provide formalised information e.g. presentations to staff meetings, staff training as well as telephone advice.

  • Coordinating a careplan to address issues where hospital staff, the patient or the relative or carer of the patient believes there are problems meeting the persons needs. Typically these issues might involve communication, behaviour or concerns around consent and capacity.

  • Helping with discharge planning, where needs have changed
