Citywide model for adult community services shortlisted for two national awards

Our innovative model for providing citywide adult community care has been shortlisted for two prestigious national awards.

The Trust has been shortlisted in the Nursing Times Awards 2013 'nursing in the community' category and in the Health Service Journal Awards 2013 'primary care and community service redesign' category in recognition of its success in developing an accessible and responsive service capable of providing high quality healthcare in community settings as an alternative to acute hospital admission.

The service provides referring professionals with 24-hour access to services such as community nursing or domiciliary therapies and also incorporates a citywide bureau handling admissions to acute and community inpatient beds.

Developed citywide over the last two years, the single point of access now handles 500 calls a week and provides rapid response care in community settings to 200 people a week as a direct alternative to acute hospital admission.

The shortlisting follows the division's success in last year's HSJ Awards when work to implement and develop the service was highly commended in the 'primary care and communiy service redesign' category.

The winners of the Nursing Times Awards 2013 will be announced on Wednesday, 30 October and the Health Service Journal Awards 2013 on Tuesday, 19 November.
