There is just one week left to nominate a staff member or team in our Summer 2012 staff awards. Nominations close at midday on Friday 1 June.

This is your chance to say a special thank you to a staff member or team that has provided outstanding care to you, your family or friends.

The Trust wants to hear about teams or individuals who, in the past year, have put patients at the heart of everything they do, whether it is listening to feedback and taking action to make a service better, or making patients feel special by showing them the utmost care and respect.

You can nominate a staff member or team for any of the six categories which match up to our corporate values  – accessible, responsive, quality, ethical, commitment and caring – plus an extra category for for partnership working (see bottom of this page for an explanatation of each category).

You can submit your nomination either:

  1. Online: click here to fill in the online nomination form, or;

  2. By phone: service users without online access can call 0121 466 7286 and submit their nomination over the phone.

 

The awards ceremony will take place at the Annual General Meeting on Wednesday 18 July at the Centennial Centre in Edgbaston. All staff that have been nominated will be informed, via email, of the status of their nomination.

 

The categories:

  • Accessible: Awarded to the individual or team that can demonstrate a change in a way of working that has improved accessibility for service users

  • Responsive: Awarded to the individual or team that can demonstrate an improvement in service user experience of our organisation through listening to their views and responding positively

  • Quality & Innovation: Awarded to the individual or team that can demonstrate quality outcomes in service delivery

  • Ethical: Awarded to an individual or team that can demonstrate actions that have helped make them openly accountable and ethical in their work

  • Commitment: Awarded to the individual or team that can demonstrate an unswerving passion to drive forward improvements for service users

  • Caring: Awarded to an individual or team that can demonstrate that through a caring and respectful approach they retain the end user at the heart of everything they do

  • Partnership: Awarded to the individual, team or service that can demonstrate improvements in service user experience or outcomes through a partnership approach.
