Concerns and complaints

The Dental Services Division is constantly working to ensure you leave your visit to Birmingham Dental Hospital or one of our community clinics feeling happy and satisfied with the service you have received.

To enable us to continue to do this by making us aware of any improvements you feel need to be made or advise us of an area of the service you feel works really well, we encourage our patients or any visitors to contact the Dental Services Patient Experience Team with feedback – both positive and negative.

Your feedback helps us to:

  • Understand what is important to our patients;
  • Support you with any difficulties you may be having;
  • Highlight which issues need to be reviewed and improved;
  • Showcase what we are doing right as individuals and as a service.


If you do wish to contact the team for support to resolve any concerns or inform us of a compliment you have regarding your care or experience, you can do so by using the contact methods displayed above.

Please note, if you are making contact with an appointment or referral related enquiry, you will be re-directed to the relevant Booking Team for either hospital or community, as they will be able to provide you with the most up-to-date information regarding clinics.

Friends & Family Forms

When attending either hospital or community clinics, you will notice that in each waiting / reception area there is a post-box with Friends & Family feedback forms for you to complete. These can be completed in relation to the experience you have had with the Dental Specialty you are visiting that day or alternatively to inform us of your feedback in relation to the wider service or environment.

How can I complete these forms?

  • You can complete a hard-copy form on the day of your visit whilst you are waiting/after your appointment and post your completed copy within one of the designated post-boxes;
  • If you’d rather take a form home to complete, please feel free to do so and post in to a post-box on your next visit;
  • You can complete the form online by clicking here. You can access the most appropriate survey for you via the link. A brief summary of each survey option is listed below:

Standard Survey

This survey is applicable for all Dental Services that operate over Birmingham, Dudley, Sandwell and Walsall.

 Accessible format

This survey is applicable for all Dental Services that operate over Birmingham, Dudley, Sandwell and Walsall. The options have been simplified for use.

Special Care Dentistry Service 

This survey is applicable for Special Care Dentistry service users and includes the clinics that offer this service across Birmingham, Dudley, Sandwell and Walsall.

Patient Engagement Forum – Get involved!

We are keen to interact with our patients and visitors through our virtual / video Patient Engagement Forum. This forum gives individuals the opportunity to express their opinions, provide vital feedback and be involved in contributing to the improvement of service delivery and therefore increase positive patient experiences.
