The service provides management of chronic kidney disease (CKD) where kidney function is moderately or severely reduced. Services offered include patient and professional education.

 

The Chronic Kidney Disease (CKD)  Service’s aim is to support the primary care teams to deliver a high class, patient centred, timely and culturally appropriate service to patients within the Birmingham area with kidney disease.

 

 

Core aims

  • To raise awareness of CKD and the health implications associated with it.
  • To support primary care teams in CKD identification and monitoring in high risk groups (e.g. pre-existing diabetes, cardiovascular disease (CVD )).
  • To provide advice and guidance to primary care, especially regarding strategies to lower CVD risk and reduce progression of CKD.
  • To triage the requirements of patients with CKD, optimising the use of resources and ensuring that care is proportionate to risk requirements.
  • To provide local community clinic reviews by kidney specialised consultants and nurses for patients with CKD.
  • To educate CKD patients and their families on how best to care for themselves.
  • To work closely with secondary care services to ensure seamless transfer if needed.
  • Where dialysis is not appropriate, to provide support for individuals and other healthcare staff to provide high quality end of life care.

 

 

Where are our clinics?

Clinics are held at :

  • Summerfield Primary Care Centre
  • Aston Health Care Centre

We also hold virtual clinics and triage sessions at different CCG Hubs and GP surgeries.

 

 

The service is Consultant led and facilitated by an Advanced Clinical Practitioner in Chronic Kidney Disease.
