Chronic Kidney Disease (CKD)

The service provides management of chronic kidney disease (CKD) where kidney function is moderately or severely reduced. Services offered include patient and professional education.

The service provides early detection and early management of Chronic Kidney disease. The aim is to support all primary carers to identify, monitor and advise those who are diagnosed with kidney disease at an early stage (KDIGO stages 3-4) to delay the progression of the condition and prevent those with modifiable causes of CKD such as diabetes and hypertension from accessing dialysis prematurely.  BP tru which is equivalent to 24 hour blood pressure monitoring day average is also undertaken during patient clinical assessments to assist with diagnosing high blood pressures. 

They also provide supportive and end of life care to those patients and their families who opt for conservative management of CKD.

Who delivers it?

The service is delivered by consultant-led, multi-disciplinary clinic, in some instances combined with the diabetes team.
