Below you will find a number of resources that will support you with the rollout of your local Integrated Neighbourhood Team.

Project plan for rollout
This project plan highlights the key activities that you will need to complete in order to implement the Integrated Neighbourhood Team in your Primary Care Network. The list is not exhaustive and you can add in any additional actions that you feel are required to rollout the model.
There are actions that will need to be completed in conjunction with the INT programme board members. The contact details for the key members of the team will be provided once you commence your rollout project.
Standard operating procedures
There are two Standard Operating Procedures (SOP) documents below.
Multi-Disciplinary Team SOP: This document will need to be reviewed by all members of the MDT team as it provides an overview of the process and also guidance on the timescales for the key actions.
Integrated Neighbourhood Team Co-ordinator SOP: This document will provide your INT Co-ordinator with the information they need to undertake their role, with an overview of the Clinical Management Team system and the administrative requirements when supporting an Multi-Disciplinary Team team.
Also included are conversation templates for the professionals within the Multi-Disciplinary Team team. These will ensure that there is consistency in the approach and that any handovers between team members and other INT teams will be smooth.

Team administration
Sustainability
The Sustainability Model will allow you to take a snapshot of the work you have done in order to understand if the changes you have implemented will be maintained over time. Your rating will be assessed against 10 key criteria for making changes sustainable.
The model will help you to:
• Better understand the key elements which can help or hinder sustainability of the team implementation
• Identify problems and issues early in your implementation and identify any areas of weakness
• Use the tool as a diagnostic when things were not going to plan and it is not clear why
• Facilitate team discussions using the model which has highlighted issues that you were not previously aware of
• Identify areas of strength within your implementation
• Monitor your progress and track improvement over time
If you have not scored well in one particular area the sustainability guide will provide some ideas of how you can make improvements.
We recommend that you complete the sustainability guide every 6 — 8 weeks until you feel that you have everything in place to make your INT a success.
