Bereavement Support

If you are reading this information, you may have had a recent bereavement. If that is the case, please accept our sincere condolences at this difficult time.

When someone dies, there are many decisions and arrangements to be made.  The information below aims to provide some useful help and advice during the early days of bereavement, including a focus on practical things you will need to do and support in understanding your emotional and physical responses to bereavement. There are also some useful contact numbers if you would like to access some further support.

Feedback on your experience of end of life care of your family member, partner or friend

Birmingham Community Healthcare NHS Foundation Trust considers End of Life Care to be a priority and receiving feedback from you helps us to understand what we are doing right and where we need to improve. We have included a link below to a questionnaire that focuses on the care that your family member, partner or friend received in their last few days of life. It also asks you to consider and feedback on your experience during that time. 

We recognise that reading the questionnaire may bring back strong memories and emotions and completing it may be difficult. Therefore, if you do wish to complete the questionnaire, please do so at a time when you feel ready.
