Welcome to the Birmingham and Solihull Community Care Collaborative.

 

Birmingham Community Healthcare Foundation Trust (BCHC) is leading the delivery of the  Community Care Collaborative programme. This is a partnership between primary care, community health services, community mental health services, social care and the community and voluntary sector.

 

It aims to provide better joined up health and care services in local neighbourhoods across Birmingham and Solihull. This will make it easier for people to access the care they need, when and where they need it and by the right person. It will also enable people to stay well for longer and more independently, in their own homes.

 

Welcome to two short videos which introduce the Community Care Collaborative. The first is the overall story of the Collaborative.  The second is presented by Richard Kirby, CEO of Birmingham Community Healthcare Foundation. Richard tells us a bit more about the work of the collaborative and how it aims to make it easier for people to access the right care they need, when and where they need it.
