Complex Neuro Rehabilitation Specialist Clinic

Outpatient service: assessment and management of significantly disabling complex and rare neurological conditions.

About our service

The Complex Neuro Rehabilitation Specialist Clinic (CNR clinic) is a multidisciplinary outpatient service that provides comprehensive assessment and management of the individual rehabilitation needs of significantly disabling complex and rare neurological conditions.


We aim to:

  •  Complete an holistic assessment by several members of the specialist rehabilitation (an interdisciplinary) team 
  • Complete a physical assessment to determine how your weakness /stiffness affect you 
  • Give an opportunity to voice concerns related to your condition 
  • Identify how your condition affects you on a daily basis, 
  • Identify realistic goals for intervention and future management 
  • Identify factors which could influence your condition in the future 
  • Make specific recommendations and agree with you referrals on to relevant services


This may also include:

  • Giving specific individual advice and information 
  • Giving condition specific education 
  • Health promotion 
  • Recommend a change in medical management
  • Giving information and advice on self-management of your condition and symptoms


Following assessment in clinic the team may make onward referrals to local services, to other specialist regional services, provide advice to your GP, complete further assessments, obtain funding for specialist equipment or provide short term therapy intervention.


Who is the service for?

  • Any individual who has or suspected of having a Long term neurological condition.
  • Lives in the West Midlands registered with a West Midlands GP, although we will assess people outside the region.
  • Aged 16 and over
  • Requires multidisciplinary assessment and management of their needs relating to their long term neurological condition
  • Their needs are not being met by other services 


Examples of conditions seen in the clinic:

  • Cerebral palsy
  • Ataxia
  • Hereditary Spastic Paraparesis
  • Vanishing White Matter 
  • Multi-System Atrophy.
  • Complex stroke, traumatic and hypoxic brain injury that have additional physical needs such as needing hoisting for transfers, PEG feeding, indwelling catheters or tracheostomy that cannot be catered for in the brain injury clinic.


These diagnoses can lead to difficulties in some of all of the following areas:

  • Muscle weakness or stiffness/spasticity 
  • Mobility and moving around 
  • Bladder and Bowel management 
  • Swallowing, Feeding and communcation
  • Pain management 
  • Activities of daily living and work

The aim of our clinic is to optimise independence and activity in social, personal and domestic life and assist people in managing the impact of their long term neurological condition.



Who delivers it?

Complex Neuro-Rehabilitation clinic appointments routinely include a:

  • Consultant in Rehabilitation Medicine
  • Specialist Neuro-Physiotherapist
  • Other team members may be involved in treatment according to individual patient problems and needs which are identified from the referral information or at subsequent appointments. This may include:
  • Specialist Occupational Therapist in Neuro Rehabilitation
  • Specialist Nurse in Neuro Rehabilitation
  • Clinical Psychologist
  • Speech and Language Therapist
  • Counsellor

  • Appointments Co-ordinator
