Welcome to Neighbourhood Health

Neighbourhood Health is a new way of delivering health and care services that puts people and communities at the heart of everything we do. It is about bringing support closer to home, helping people stay healthy, independent and connected to their local community for longer.

 

Making support easier to access, closer to home

Across Birmingham and Solihull, health services, councils, social care teams and voluntary and community organisations are working together to provide more joined-up support in the places where people live.

 

Good health is about more than just healthcare. It is influenced by many aspects of daily life, including housing, employment, social connections and access to local support. By working together and sharing expertise, organisations can better understand local needs, reduce health inequalities and make sure people receive the right support at the right time. This includes listening to communities whose voices may not always be heard and ensuring services are shaped around what matters most to local people. Neighbourhood Health Centres will bring many of these services together in local communities, making care more convenient, connected and easier to access.

 

How Neighbourhood Health supports you

Neighbourhood Health focuses on three key areas. Proactive Care helps people stay well and manage long-term conditions before problems become more serious. Access to Urgent Care ensures people can get timely support when they need it, helping to prevent some health concerns from becoming emergencies. Flow and Capacity supports people to receive care in the most appropriate setting and return home safely and quickly when hospital care is no longer needed. Integrated Neighbourhood Teams, made up of professionals from health, care and community organisations, are central to making this happen.

 

Building on a strong foundation

Neighbourhood Health has evolved from the Birmingham and Solihull's Community Care Collaborative (CCC). The CCC, led by Birmingham Community Healthcare (NHS) Foundation Trust (BCHC) worked with health and care partners to deliver more joined-up, neighbourhood-based care and support people to live well for longer in their own homes.

 

BCHC has become a core delivery and strategic partner of Neighbourhood Health which is being led by University Hospitals Birmingham NHS Foundation Trust.

 

Working together for better health

Neighbourhood Health supports the Government's vision to move care from hospitals into communities, focus more on prevention and make better use of digital technology. By working together, we can help more people live healthier, happier and more independent lives, while reducing health inequalities and improving care for everyone.
