Double award recognition for Community Care Collaborative projects

On behalf of Community Care Collaborative partners, Birmingham Community Healthcare is proud to announce that key projects in our health and care transformational programme have been shortlisted in the 2025 HSJ Awards.

Our Integrated neighbourhood teams model has been selected for the category of primary and community care innovation of the year and our locality and neighbourhood health model has been chosen for the data-driven transformation award.

This recognition highlights the exceptional work and innovation demonstrated by the team and is testament to the dedication of the CCC partners in driving forward health and care excellence.

The awards ceremony takes place in November 2025.
