Direct Access to Wellbeing Services Privacy Notice

This Privacy Notice explains how Birmingham Community Healthcare NHS Foundation Trust (BCHC) Charity collects, stores, uses, protects and shares personal information when you access or receive support from the Direct Access to Wellbeing Services (DAWS) Team.

 

The Direct Access to Wellbeing Service is fully funded by Cadent under the Vulnerability and Carbon Monoxide Allowance (VCMA) and Cadent Foundation until 31st March 2026. 

 

The VCMA as derived from Ofgem is awarded to each Gas Distribution Network to undertake projects to offer support to individuals living in vulnerable situations.

Who we are

BCHC Charity is the Data Controller for personal information processed and are an official charity registered with the Charity Commission under the Registered Charity Number 1069427.

 

The Direct Access to Wellbeing Services (DAWS) Team are a part of the BCHC Charity Team and offer services in the following areas: 

  • Health and wellbeing guidance and signposting including early health messages such as healthy eating, oral health and falls prevention.
  • Awareness of BCHC health services and the wider NHS Support available.
  • Energy saving tricks and tips and provision of energy saving equipment (where applicable).
  • Signposting for those living in fuel poverty and resources to support (where applicable).
  • Simple benefit checks and signposting for further support.
  • Registration onto the Priority Service Register (PSR) (where applicable).
  • Provision of carbon monoxide (CO) alarms and advice regarding CO.

 

The main aim of the DAWS team is to support those most vulnerable and in need in local communities and to ensure they have access to wider support available.

Personal data we collect about you and how it is used

The data we collect about you may include, but is not limited to your:

  • Full name.
  • Contact information including email address, postal address, telephone number.
  • Age.
  • Date of birth.
  • Gender.
  • Health data including pre-diagnosed health conditions where exacerbation may take place due to vulnerabilities. Details of physical or mental health condition that increase the service users' vulnerabilities in their home such as relying on medical equipment, safety related, mobility related, communication need, developmental conditions.
  • Equal opportunities monitoring information- race or ethnic origin, (you can select option 'prefer not to say' if you do not want to provide this data).

 

Your personal data is collected via a referral form and is used for administrative purposes to provide and facilitate comprehensive support to you. This may also include sharing your data via an onward referral to partner agencies (with your consent).

The legal basis for processing your personal data

We process your personal data on the basis that you have given explicit consent for one or more specified purposes in accordance with (Article 6(1)(a) UK GDPR).

 

Special Category Data 

We may collect your health data and data about your race and ethnic background.

 

This processing is carried out with your explicit consent and in accordance with Article 9(2)(a) of the UK GDPR.

 

Health information- data collected about your physical or mental health condition is used to help us understand your needs and to inform the support or services we can offer you.

 

Race and Ethnicity

This data is collected to:

  • Promote equality, diversity and inclusion in line with the Equality Act 2010.
  • Identify and address any disparities in access to services or outcomes.
  • Ensure our services are fair, inclusive and responsive to the needs of all communities.

Who we share your personal data with

Our Community Wellbeing Officers (CWO's) have links to experts in the local area and the additional services that are being provided by our partners across the city. If an individual needs information or additional support outside of what the DAWS Team can offer, our CWO's will be able to signpost individuals to the relevant services or complete an onward referral to partner agencies (with your consent) so that you can access additional support.

How long do we keep your personal information for?

We keep your personal data for 12 months following support we have offered to you or following an onward referral to one of our partner agencies. After this period your data will be deleted securely.

What measures do we take to protect your information?

We take our duty to protect your personal information and confidentiality seriously. We are committed to taking all reasonable measures to ensure the confidentiality and security of personal data for which we are responsible for.

 

We store all personal information on a dedicated, protected database.

Transfer of data outside the European Economic Area

We will not transfer your data outside of the UK.

Your rights

Under data protection law, you have the following rights:

  • Your right to withdraw your consent: you can withdraw your consent at any time. This means we will stop processing your personal data unless we have another lawful basis to do so.
  • Your right of access: you have the right to request a copy of information we hold about you.
  • Your right to rectification: you have the right to ask us to correct information that is factually incorrect, or to include anything that is missing.
  • Your right to erasure: you can ask us to delete your personal data, especially if you withdraw your consent or the data is no longer needed.
  • Your right to restriction of processing in certain circumstances.
  • Your right to object to processing in certain circumstances.

Use of automated decision-making or profiling

The personal information you provide to us is not used for:

  • Automated decision making (making a decision by automated means without any human involvement).
  • Profiling (automated processing of personal data to evaluate certain things about an individual).
