About our service
The Division provides a Single Point of Access (SPA) as the primary point of contact for all referrals to the LD Service during core working hours; referrals are prioritised based on level of need and urgency.
Referrals
Respite and Day Services eligibility criteria
- Aged 17 ½ years and over.
- Diagnosed learning disability who needs specialist health support that cannot be met through mainstream services even with the use of reasonable adjustments.
- Continuing Healthcare Criteria (CHC) funded: must be assessed as eligible for full CHC funding with written proof.
9 Kingswood Drive eligibility criteria
- Aged 18 years and over.
- Diagnosed learning disability who needs specialist health support that cannot be met through mainstream services even with the use of reasonable adjustments.
- Continuing Healthcare Criteria (CHC) funded - must be assessed as eligible for FULL CHC funding with written proof.
- On the End of Life pathway.
All other Community Services eligibility criteria
- Aged 19 years and over (or within 6 weeks of their 19th birthday).
- Diagnosed learning disability and needs specialist health support that cannot be met through mainstream services even with the use of reasonable adjustments.
- Registered with a Birmingham GP.
- Moving into the Birmingham area with a Birmingham residential postcode and an identified Birmingham GP, but are unable to register with the GP until they have moved into their property (for example, a service user discharged from hospital or a complex case moving into the area and going through the transfer or handover of care process). Confirmation will be required for service users eligible for s117 that the transferring Learning Disability Service have had a conversation with the local Integrated Care Board (ICB).
Triage
All referrals undergo an initial triage process. Following this, there are weekly Multi Disciplinary Team (MDT) meetings held to discuss and decide the specific outcomes of all new referrals to the service. These could include:
- Accepting the referral and deciding which team will make first contact with the referrer/service user.
- Assessing that further information is required to enable an informed decision. The referrer will be contacted accordingly.
- Signposting the referrer to a more suitable service if the referral does not meet our eligibility criteria.
