Once a referral is received by the Single Point of Access (SPA) team, the Triage team are then notified and they will screen all referrals to assess their eligibilty, based on core access criteria.
Triage process
- Once a referral has been assessed as meeting the service criteria, the reason for referral will then be assessed for urgency, based on risk and referral need.
- A member of the Triage team will make contact with the service user, carer(s) and / or service that has referred the service user in order to complete a holistic assessment - this allows the service user's needs to be clearly identified. All information is recorded on the service user's electronic care record.
- The information thus gathered is discussed at a Multi-Disciplinary Team (MDT) meeting, where clinical professionals discuss the referral reason and make a joint decision on what the Learning Disability service can offer, based on the needs identified during the assessment process.
- There may be occasions where the MDT require further information before a decision can be finalised. In such instances, a request will be made either by phone or in writing to the most appropriate person involved with the care of the service user.
- Once a final decision has been made, the referrer, the service user and their GP are all notified of the outcome of the MDT discussion by post or email.
- If, however, a service user does not meet the Learning Disability service criteria, we will aim to provide suitable signposting information to other services which may be able to support the health needs of the service user, as outlined in the original referral.
