A city-wide day service for individuals with a Learning Disability who, in addition, also have complex needs.

About our service

Learning Disability Services provide a city-wide day service for individuals with a learning disability who, in addition, also have complex needs.  These needs may include behaviours which may challenge or profound multiple disabilities.  The Day Service unit is based at Riverside Lodge in Small Heath.
 

The Day Service unit provides support in such areas as:

  • Specialised assessments in regard to health care needs, communication skills, engagement and behavioural needs.
  • Formulation of interventions and measurable outcomes.
  • Offering engagement in meaningful activities both in-house and in the community. This could be on a 1:1 or group basis.
  • Support to develop skills and abilities.
  • Support with developing communication and making choices.
  • Support with managing complex health conditions such as Dysphagia, Diabetes and Epilepsy.
  • Health monitoring and support to attend mainstream health appointments if required.
  • Formulation of Positive Behaviour Support Plans and Quality of Life plans.
  • Regular reviews and evaluation of progress.

 

 

Referrals

Eligibility criteria:

  • Aged 17 ½ years and over.
  • Diagnosed learning disability who needs specialist health support that cannot be met through mainstream services even with the use of reasonable adjustments.
  • Continuing Healthcare Criteria (CHC) funded: must be assessed as eligible for full  CHC funding with written proof.

 

A referral to the service can be made through a number of means including a patient’s GP, carer, or family member.
