Inpatient units at Moseley Hall work within the discharge to assess [D2A] model, using a ‘home first’ approach. Our aim is to plan your discharge within 21 days.

About our service

We are a 28-bedded stroke rehabilitation ward that provides intensive, multidisciplinary therapy and medical care to help patients recover independence after a stroke based at Moseley Hall Hospital in the South of the city.

 

What happens on a stroke rehabilitation ward?

  • Multidisciplinary Care: Patients are supported by a team including Doctors, Nurses, Physiotherapists, Occupational Therapists, Speech and Language Therapists, and Dieticians.
  • Goal-Oriented Therapy: therapy sessions focus on mobility, swallowing, communication, cognitive skills, and daily activities like dressing or washing.
  • Moseley Hall works within the  Discharge to Assess (D2A) Model , using a 'home first' approach. The ward teams aim is to begin discharge planning with the patient and their family on admission to the wards. Early discharge planning should ensure that when inpatient care is no longer required the necessary arrangements to return home or to a 24-hour placement are in place.
  • In accordance with the 'Discharge to Assess Model' our aim is to get patients to their appropriate discharge destination as soon as they are medically optimised and no longer need to be in hospital.
  • Whilst rehabilitation begins in hospital this can be continued within the community, therefore avoiding keeping patients in hospital solely for this purpose.
  • There is a set criteria for admission and the referring hospital or team will liaise directly with MHH, ensuring a seamless handover of care.

Visiting

We encourage open visiting and only ask that you avoid mealtimes:

  • 12 p.m. to 1 p.m.
  • 5 p.m. to 6 p.m.

This allows people to have their meals without interruption as nutrition is a vital part of their health, wellbeing and recovery from illness.

If you decide to visit early in the morning (8-10 a.m.) please be aware that there will be limited availability of professional colleagues to respond to your queries.

We can only support two visitors per bedspace, but communal areas of the hospital are available for larger groups of people. Children are permitted to visit at your own risk but must be supervised by an Adult at all times.
